ORDER MATERIALS FOR YOUR PATIENTS
All the patient materials on our website are free to download. All you need to do is open them as a PDF and print them off. However, we are delighted to offer hard copy versions of patient materials FREE OF CHARGE, so that they are available in your surgery or to give to your patients. Materials and delivery are free of charge while stocks last.

Please note that there are some differences between the downloadable and hard copy materials:
- If you order the ‘Tailoring dietary advice for lowering cholesterol’, ‘Moving to a lower saturated fat diet’ and ‘Secondary care cardiac rehabilitation’ leaflets, they will come in a tear-off pad format. Each pad consists of 50 tear-off sheets to make it easier for you to share the information with your patients.
- If you order the ‘Swap to healthier foods’ chart, you’ll receive it in a handy wheel format. The wheel is an interactive tool which recommends healthier alternatives to foods that are high in saturated fat.

This Short Film Could Save Your Life (DVD)
This DVD is designed to communicate the risk of cardiovascular disease, the importance of maintaining good heart health, and the role plant sterols and stanols play in managing cholesterol levels. You can use it to educate groups of patients (in cardiac rehabilitation meetings, for example) or as an informative and engaging video to show in your waiting room.
activheart Healthcare Professional support packs, Primary Care Trust support packs and Registration pads
activheart is a free online programme designed to help your patients improve or maintain their heart health. It acts as a virtual coach, helping patients bring new positive behaviours into their daily lives by providing on-going psychological and social support. Whether you’re a healthcare professional or work for a Primary Care Trust, these packs have all you need to get started. You can also use the form to order any additional copies of the activheart Registration pads (with 50 tear-off sheets per pad)
